- Complete the authorized Dealer Application form.
- Return the Application form and a copy of your W-9 form via fax (678-405-6081) or email to dealers@polarisusa.com.
You’ll receive a telephone call from a PolarisUSA Video representative to review your application, discuss the business opportunity and to establish how to proceed to make your experience a business success.
